We require the following documentation and medical results:

Infection parameters
    • HBs-antigen (hepatitis B)
    • Anti-HBc-Ab (IgG) (hepatitis B) antibodies
    • Anti-HCV-Ab (Hepatitis C) antibodies
    • Anti-HIV 1, 2 antibodies

Further results:
    • Blood type + rhesus factor

    • Estradiol (E2)
    • Progesterone
    • Free testosterone
    • DHEAS
    • Prolactin
    • LH
    • FSH
    • TSH
[Please let us know, whether you take any medicaton (e.g. contraceptive pill) and inform us on the day of your menstrual cycle on which  the blood sample was taken.]

Please note:
nfectious disease screening should not be longer ago than 1 year.
All laboratory findings have to be provided in writing being confirmed by a physician or a laboratory. 

If you opt for the preservation of our oocytes, we will need the signed information sheet, consent form and treatment contract as well as a copy or your passport or identity card prior to the start of your treatment.
